TMC Hospice
TMC Hospice is a local nonprofit serving the community since 1991. We are accredited by the Joint Commission, certified by Medicare and are a Level III partner of We Honor Veterans.
Hospice cares for people with any terminal illness or injury after the patient, physician and family decide that aggressive curative treatment is no longer appropriate. Hospice care provides a sensitive support system that helps patients and their families cope with physical, spiritual and emotional concerns related to the terminal condition.
Using an interdisciplinary group approach of physicians, nurses, social workers, home health aides, volunteers, spiritual counselors, bereavement counselors and others, we provide palliative care in the home, short-term inpatient/respite services, mobilization and coordination of ancillary services and bereavement support.
When a patient is ready for comfort care, choosing hospice care means avoiding unwanted hospitalizations and medical treatment and procedures. The TMC Hospice Team aims to support patients' and caregivers' wishes while helping them achieve their goals of care.
