Private Duty assistance is typically provided to elderly individuals who need help with day-to-day activities. Housekeeping, meal preparation, medication reminders, and personal care such as bathing, dressing and hygiene are all tasks that your Private Duty Aide can assist you with!
Transitional Care is a service that can help transition a patient from one care setting to another. This may include coming home after a hospital stay, moving into a new facility, or even moving into a loved one’s home. This type of care helps not only you or your loved one, but also provides a peace of mind so no one has to worry about missing any medications, information, forms, or doctor’s appointments needed. Our Transitional Care Program is guided by a Licensed Medical Social Worker alongside a Registered Nurse.
Palliative care is specialized medical care for people living with a serious illness. The Palliative Care teams focus on providing relief from pain, stress, and symptoms of life-limiting illness while improving quality of life from the comfort of your own residence.
Patients and their families call on hospice care when curative treatments are no longer desired. Quality of days instead of quantity of days has become the new healthcare goals at the hospice stage. Hospice teams provide compassionate care at the final stages of life to those with incurable conditions so they may live as comfortably as possible.
